Grant Process

There are four steps to granting Hope. Each step builds on the next. A LofLC grant is frequently a source of relief for patients and families undergoing difficult medical treatments and a positve force that helps them overcome their obstacles. A grant sometimes helps rekindle a patient belief in themselves and promise of hope in their future.

Four Steps For a LofLC Grant!

Referral

1We rely on family members, friends, medicals professionals, for most of our referrals. We understand that most patients are too emotionally drained during their treatment to cope with the grant process.

Medical Eligibility

2We determine a patients medical eligibility with the help of the treating physician. To receive a LofLC grant the patient must be diagnosed with any type of cancer and in the process of treatment.

Accessing Type of Grant

3We send out one of our enthusiastic volunteers to learn more about patient and their needs.These passionate volunteers connect with the patient and family and help in completing the grant process.

Grant Approval"

4Once the grant is approved our volunteer will cantact the patient or family again and begin making it happen. The Volunteers will stay in touch with the family or patient throughout the grant implementaion process.

Verification Process
What is the verification process of receiving a grant?.

1. Patient, family members, caregiver, or friend have to fill out the applicatoin form and send by USPS mail or email, fax or online.
2. Patient physician has to fill out our verification form and fax or mail to our offices. No email allowed.
3. LofLC of Lea County volunteer visits the patient residence for a signature and assessment of grant needs.
4. Patient info is verified and accessed for type, amount of funds,duration of assistance needed by the grant committee.
5. Patient or caregiver is contacted with the grant information and processed once the approval has been completed.
